((
At 54 Linda had classic symptoms of stress urinary incontinence...and it was starting to interfere with her life. LINDA KLEINER/PATIENT: WHEN I WOULD COUGH WHEN I WOULD LAUGH I WOULD LOSE URINE. IF I WOULD BEND DOWN TO PICK SOMETHING UP I FOUND MYSELF LOSING URINE. I ALSO FOUND IT WAS HARDER TO KEEP MY URINE I NEEDED MORE AND MORE TRIPS TO THE BATHROOM AND IT BECAME VERY UNCOMFORTABLE AND EMBARRASSING.
Unlike Linda, doctors estimate that about half of all women are too embarrassed to seek help. The problem occurs when the pelvic floor becomes weak usually from childbirth. Treatments have been available for quite some time...including exercises, medication and surgery done either through the abdomen or through the vagina. DR. ROBERT SALANT/UROLOGIST: THE SURGERIES HAVE BEEN RELATIVELY SUCCESSFUL IN THE SHORT RUN,
BUT THE LONG TERM SUCCESS HAS BEEN QUITE POOR---30-45%.
One of the reasons is---doctors used the abdominal wall to secure

sutures. A new procedure uses a different approach to treating the problem which occurs when the bladder falls through the pelvic floor. called the Vesica Procedure--doctors insert two small titanium screws into --the pelvic bone--, and using sutures--pull the urethra and the bladder up, so pressure from a cough or sneeze won't cause urine to leak. DR. SALANT: THIS YIELDS A MUCH STRONGER SUPPORT MECHANISM FOR STABILIZING THE URINARY BLADDER.
This technique has a couple advantages over older treatments for stress incontinence. First of all no major surgery is required just too small incisions.
Patients can usually go home the same, and the recovery is speedy with very minimal pain. DR. SALANT: IT IS SURPRISING HOW WELL THESE PATIENTS HAVE DONE WITH THIS PROCEDURE. I HAVE DONE SEVERAL HUNDRED OF THE OLDER TYPE OF ENDOSCOPIC PROCEDURES AND IN GENERAL PATIENTS TEND TO COMPLAIN FOR SEVERAL WEEKS OF LOWER ABDOMINAL PAIN AND PULLING.
Though the procedure is relatively new, recent studies have shown it corrects the problem 94-percent of the time. And researchers believe that it is a permanent solution to stress incontinence and so far Linda couldn't be happier with the result. LINDA KLEINER:I NO LONGER LOSE MY URINE WHEN I'M LAUGHING WHEN I SNEEZE AND CAN BEND DOWN AND PICK THINGS UP. I'M NOT RUNNING TO THE BATHROOMS I WAS BEFORE AND I FEEL VERY FREE.
With such a high success rate in the latest studies, the vesica procedure is likely to become the most common way this problem is treated.
