CANADIAN REPORT NEVER SENT TO U.S.

Canadian officials said Monday they never sent their U.S. counterparts a preliminary report into a 1989 crash under conditions similar to those of the recent USAir crash at New York's La Guardia Airport.

Last week, the head of the Canadian inquiry implied that the March 22 crash at La Guardia might have been avoided had U.S. officials heeded his panel's report, which U.S. officials said they never received.

Steady snow was falling when USAir Flight 405 crashed on takeoff, killing 27 people and injuring 24. The plane, a Dutch-made Fokker 28, had been de-iced twice before takeoff with an alcohol-based solution called Type 1. The second de-icing occurred 30 minutes before takeoff.

No official determination has been made, but there have been reports that ice on the wings contributed to the USAir crash.
