Report alleged incidents of resident abuse and injuries to the Office of Health Facilities Regulation.

Implement an every-other-day bathing schedule. Residents who require shaving will be shaved every morning.

Train workers on residents' right to dignity. Workers are not to borrow personal possessions or remove furnishings from individual rooms.

Order towels, bibs and appropriate chairs for residents' needs.

Remove linen, medication and treatment carts from residents' activity room and store in newly purchased storage building.

Institute new cleaning practices to reduce offensive odors.

Fix malfunctioning air conditioners and toilets.

Assess every resident for pressure sores and train nursing personnel on procedure for dressing wounds. Install whirlpool.

Retrain personnel on responsibility to report injuries or change in condition.

Maintain proper documentation on residents' fluid intake.

Revise staff assignments to maximize care-giving.

Monitor call lights on every shift to ensure they are within reach of residents and operational.

Contract with new pest company.

Train staff in handling emergencies. Provide proper equipment and train staff on sanitation procedures for used emergency equipment.
