Who's eligible for refund: Any Blue Cross-Blue Shield policyholder who made a co-payment for medical services in the years 1984-1993. This includes those who belonged to the Roanoke plan before its merger with Richmond in 1986.

Deadlines: Claims must be filed on a proof-of-claim form by Dec. 7.

How to find out more: The company will notify potential claimants by letter and in newspaper advertisements, but policyholders who have questions about whether a particular facility is covered may call (800) 422-4662.
