Trigon Blue Cross Blue Shield will extend by a month the deadline for policyholders to apply for coinsurance refunds under a state-ordered program announced in September. The new deadline is Jan. 7.
The company will inform customers of the new deadline in newspaper advertisements to appear Wednesday.
The ads feature a full-size, cut-out refund form and point out that Trigon customers need to fill in only four blanks on the form: name, address, policyholder identification number and signature. Trigon staff will conduct the research necessary to process the claims.
Policyholders also may continue to use the original pre-printed refund forms, which are available by calling toll-free to 1 (800) 422-4662.
Through Sunday, Trigon had received 79,292 refund claim forms.
The refund program resulted from a state investigation of how Trigon computed policyholder copayments after it negotiated discounts from health care providers. The refunds apply to inpatient and outpatient services at hospitals, skilled-nursing facilities, home health agencies, substance abuse centers and the like between 1984 and 1993. It does not apply to charges for physicians, medical equipment, prescription drugs or dental services.

Bank of Buchanan branch in Troutville
Bank of Buchanan said Monday it has received approval from the Virginia Bureau of Financial Institutions to establish a branch at Lee Highway (U.S. 11) and Mountain Pass Road (Virginia 652) in Troutville.
Watts Steger, president of the bank, said it will erect a temporary modular branch at the site within 30 days.
Bank of Buchanan has assets of $68 million, 35 employees and other locations at Buchanan, Daleville and Eagle Rock.
