GUIDE TO RETIREMENT, LONG-TERM CARE FACILITIES

Following is a list of some retirement and nursing facilities in the Roanoke Valley.

Avante at Roanoke: 324 King George Ave. S.W., Roanoke, 345-8139. 141 beds, 20 of which are Medicare-certified for skilled care at $120 per day. 121 beds licensed for intermediate-level care at $80 per day for semiprivate rooms; $93 per day for private rooms. Base rate includes 24-hour licensed nursing care, incontinent care, three meals daily planned by registered dietician, assistance with feeding, certified activities director, social worker, linens, housekeeping, wheelchairs. Personal laundry service $31 a month. On-site physical, occupational and speech therapies. Chaplain on call. Dentist and podiatrist on staff. Medicare and Medicaid accepted. The Berkshire Health Care Center: 705 Clearview Drive, Vinton, 982-6691. 180 beds, intermediate and skilled care. Intermediate private room $90 per day; semiprivate room $84 per day; four-bed ward, $78. Skilled-care semiprivate room, $100 per day. Fee includes room and meals, nursing care, housekeeping, dietary and social services, maintenance and activities. Restorative aides, pharmacy, medical care and laundry at extra cost. Patients must qualify for nursing facility care and be admitted by an attending physician. Private pay and Medicaid accepted for nursing facility care. Brandon Oaks: 3804 Brandon Ave., Roanoke, 776-2600. Life-care retirement community, sponsored by Virginia Lutheran Homes Inc. ELCA, and open to all religious faiths. Facilities included independent living apartments and cottages, assisted living accommodations and a fully-staffed health center.

Independent living: Apartments and cottages, main dining room, auditorium, theater, chapel, beauty and barber shop, private dining room and wellness clinic. Flexible dining plan, housekeeping and linen services, scheduled transportation, maintenance, 24-hour security, activity program, chaplain. Entrance and monthly fees for life-care program.

Assisted living: Private accommodations with safety-equipped baths, staff on duty 24 hours. to assist with bathing, grooming, dressing and other activities. 24-hour security, emergency response system, three meals daily in dining room with special diets if needed, medication monitoring, housekeeping and linen services, activity program, scheduled transportation, chapel, lounge, activity and game room, beauty salon and barber shop. Monthly fee $2,244.

Health center: 776-2616. State-licensed 62-bed nursing facility with licensed nursing staff on duty 24 hours a day. Full range of activities and social services offered. Scheduled transportation available. Semiprivate room, $89.50 per day; private room $95 per day. Private pay and Medicaid approved.

Burrell Nursing Center: 611 McDowell Ave. N.W., Roanoke, 224-3800. 90 nursing home beds offering specialized care in skilled, ventilator-support and comprehensive rehabilitation services. Provides housekeeping and laundry (linens and personal); transportation to all appointments; full-time activities; full-time social worker; daily physical, occupational and speech therapies; kidney dialysis; dietitian on contract; beauty and barber services; on-site 24-hour nursing care by R.N.s, L.P.N.s and certified nursing assistants. Rates start at $79 per day for nursing home unit and are based on needs of patient. An affiliate of Carilion Health System.

Camelot Hall of Salem: 1945 Roanoke Blvd., Salem, 345-3894. Owned and operated by Medical Facilities of America. 240 beds, offering skilled and intermediate-level care. Medical, dental, podiatric and optometric care; physical, speech and occupational therapies on site. Private rooms $88 per day, semiprivate rooms $81 per day, skilled care $97 per day. Daily rate includes 24-hour nursing support, social services staff support, daily group and individual activities, meals planned by dietician. Laundry, medical and pharmacy services at additional charge. Residents must qualify for nursing care and be admitted by attending physician.

Cave Creek: 2140 Lee Highway, Troutville, 992-4599. Residential care and assisted living facility owned and operated by registered nurse with a 20-member staff. 24-hour supervision, private and semiprivate rooms with bath; maid service, laundry and 3 meals daily; aides accompanying residents to medical appointments; in-house recreation and outings. $950-$1,800 per month based on level of care.

Edinburgh Square: 129 Hershberger Road N.W., Roanoke, 366-1888. 135 independent-living apartments, one-bedroom and studio apartments, some apartments equipped for handicapped. No cleaning, dining or nursing service. Each apartment is self-contained. Trash chutes and laundry rooms on each floor. On-call maintenance service. Ceramics and community room. Pets allowed with deposit. Section 8 subsidized housing, meaning that rent is based on income; rent and utilities not to exceed 30 percent of adjusted income. To qualify, income cannot exceed federally established income limit for the area.

Elm Park Estates: 4230 Elm View Road (across from Tanglewood Mall), Roanoke, 989-2010. Retirement community of 110 apartments offering on-site residential managers, three meals per day, housekeeping, linens, utilities (except telephone), satellite TV, daily activities, scheduled transportation, and valet parking. Studios, one- and two -bedroom units. Each apartment has a kitchenette, refrigerator, intercom and emergency call system. Small pets welcome. Rents start at $995 per month.

Fairington Apartments: 4922 Grandin Road, Roanoke, 989-7731. Section 8 government-subsidized independent-living community for persons at least 62, or handicapped or disabled. One-bedroom apartments, some wheelchair accessible. Rent based on 30 percent of monthly income for applicants who meet HUD requirements.

Friendship Manor: Main office, 327 Hershberger Road N.W., Roanoke, 265-2100. Complete retirement community with independent living units, a home for adults providing supervised residential care and a health care center with skilled, intermediate and subacute nursing care.

Independent living accommodations: 371 units in the form of houses, town houses, apartments. Monthly rentals range from $256 a month for an efficiency to $784 a month for a three-bedroom unit, with 22 different floor plans and prices. Also, accommodations with all meals included for $765 a month. No entrance fees.

Home For Adults: Accommodates 237 people at Lakeview and Hershberger Road campuses. Offers Alzheimer's care. Semiprivate rooms range from $795 to $860 per month; private $970 to $1,485 per month depending on location and care. All meals included.

Health Care Center: 373 beds. Offers skilled, intermediate and subacute care; a diagnostic center; physical, speech and occupational therapies; dialysis center; and Alzheimer's care. Semiprivate rooms are $88 to $100 per day, private rooms are $96 to $108 per day, depending on care.

Other services: Alzheimer's care units; transportation; residents' center with indoor pool and exercise area; Manor Care Plus (personalized care in a Manor room); Care Unlimited (homemaker health-care service available to persons confined at home); temporary care at home or at Friendship Manor; physical and occupational therapy for children and adults on an out-patient basis.

Hughes Bros. Adult Care Residence: 1101 Orange Ave. N.W., Roanoke, 345-4675. Intimate 14-bed facility providing supervised residential care to ambulatory patients, 18 years or older. Private and semiprivate rooms, common dining and activities areas. Fee includes room, three meals daily, linens, weekly housekeeping, local phone calls, personal maintenance, activities and transportation to medical appointments. Private pay and Medicaid. Fees start at $675 per month.

Melrose Towers: 31st Street and Melrose Avenue N.W., Roanoke, 983-9281. Independent living apartments for people at least 62, or handicapped or disabled. Studio, one- and two-bedroom units; some wheelchair accessible. Heat, water and electricity included. Rent is based on income and other factors. Washer and dryer on each floor. On-site manager and staff, routine maintenance, 24-our emergency maintenance service.

Morningside Manor: 13th Street and Melrose Avenue S.E., Roanoke, 983-9281. Independent living apartments for people at least 62, or handicapped or disabled. Studio, one- and two-bedroom units; some wheelchair accessible. Heat, water and electricity included. Rent is based on income and other factors. Washer and dryer on each floor. On-site manager and staff, routine maintenance, 24-hour emergency maintenance service.

Our Lady of the Valley: 650 N. Jefferson St., Roanoke, 345-5111. Non-profit facility sponsored by Catholic Diocese of Richmond. 85 assisted living apartments and 30 nursing home beds. Around-the-clock nursing care, three meals daily, weekly housekeeping and laundry of linens, van transportation and planned activities. No entrance fee. Apartment fees start at $1,496 per month. Open to all religious faiths.

The Park - Oak Grove: 4920 Woodmar Drive S.W., Roanoke, 989-9501. Locally owned rental retirement community with 93 apartments. Independent living with assisted living services optional, including medication management and personal care. No entrance fee, month-to-month lease. Weekly housekeeping, linen laundry service. Three meals daily in dining room, van transportation, planned activities, security monitors, fire-safe construction, intercom and emergency call system. R.N. supervised wellness program with 24-hour on-site staff. Pets under 30 pounds welcome. All utilities except telephone included in monthly fee. Studios, one-bedroom, two-bedroom units from $1,243 per month.

Raleigh Court Health Care Center: 1527 Grandin Road S.W., Roanoke, 342-9525. 120 beds. 16 beds Medicare certified. Base rate includes 24-hour nursing care, three meals daily planned by registered dietitian, basic nursing care, wheelchairs, feeding, full-time activities and social services director, housekeeping. Semiprivate room in intermediate care, $89 per day; private room, $93. Skilled semiprivate room, $108 per day. Planned activities and outings. Patients must be certified by physician as requiring nursing care. Corporate office is Medical Facilities of America, 2917 Penn Forest Blvd. S.W., 989-3618.

Richfield Retirement Community: 3615 Main St., Salem, 380-4500. Pharmacy, clinic, chapel, social rooms, recreation service and equipment on premises. Guest apartments available. Bus service to downtown Salem and Roanoke shopping centers. Registered nurse on duty five days a week. Several types of facilities and levels of care. Meals available in the dining room and cafeteria at Richfield Nursing Center.

Ridgecrest Apartments, 3720 Knollridge Road, 380-4699. Federally subsidized (HUD) development for low-income residents. Fees are based on 30 percent of adjusted gross income. 106 independent-living apartments. Studio, one- and two-bedroom units.

Knollwood Apartments, 3730 Knollridge Road, 380-4900. 79 independent-living apartments. Six studios, 33 one-bedroom, 12 two-bedroom/one bath, 12 two-bedroom/two bath, 16 two-bedroom/two-bath deluxe apartments. Rental option that ranges from $466 to $1,065 a month. Life-lease program whereby a fee is paid from which 10 percent is deducted each year of residency. A monthly service fee also is charged to cover utilities, lawn care and security. Life-lease fees and monthly service fees range from $28,300 plus $183 per month for a studio to $69,200 plus $373 per month for a two-bedroom/two-bath deluxe unit.

Richfield Lake Estates, a cluster-housing development offering two bedrooms and two baths. Life-lease fee, $103,950 initial investment with monthly service fee of $450; a 90 percent refund plan with $144,375 initial investment with monthly service fee of $640; or monthly rental of $1,275.

T. Stuart Payne Center for Assisted Living, 3719 Knollridge Road, 380-5500. An 82-bed assisted-care facility. 48 private and semiprivate rooms are available. Three meals per day in the cafeteria are included. A registered nurse is on duty during the day, and licensed practical nurses are on duty at all times. Monthly fees for semiprivate rooms are $1,430 per month, $1,977 per month for private rooms. Respite care is available for up to 30 days at $50 per day.

The Oaks at Richfield, 3706 Knollridge Road, 380-6544. A 100-bed assisted-care facility. Three meals per day. Dining room, medications administered, assistance with personal care provided. Monthly fee for private rooms is $2,068; semiprivate rooms are $1,673; respite care for up to 30 days at $60 a day semiprivate rooms; $75 a day private rooms.

Richfield Nursing Center, 3615 W. Main St., 380-4500. 327 beds, of which 50 are certified for skilled care and the remaining are intermediate care. There are 15 private rooms at $97 per day and 156 semiprivate rooms at $87 per day; includes room and board and 24-hour nursing care. Medicaid approved.

Roanoke City Nursing Home: 4365 Coyner Springs Road, Roanoke, 977-1018. Intermediate-care nursing facility with 58 beds. Provides room, regular and special diets, housekeeping, basic laundry and personal hygiene items, regular medical and nursing care, activities and social services. Licensed by the Virginia Department of Health. Medicaid accepted. Cost is $83 per day. Preference is given Roanoke residents.

Roanoke United Methodist Home: 1009 Old Country Club Road N.W., 344-6248. A continuing care retirement community available to persons of all faiths. Private-pay arrangements only. Guest rooms, transportation services for outings and overnight trips, chapel, social rooms and library on premies.

Independent apartments: Each has outside entrance. Four one-bedroom and 22 two-bedroom apartments. Meals are not included, but are available. Stays of up to 30 days per year in the health care center are included. Emergency call included.

Catered Independence: 27 private rooms/suites for individuals and couples. Stays of up to 30 days per year in the health care center are included. Three meals per day, weekly housekeeping and linen-laundry service included. Personal laundry service optional. Emergency call in bedroom and bathroom.

Assisted living: Private and semiprivate rooms. Respite care available as vacancies allow. Registered nurse on duty 24 hours a day. Meals, laundry (linens and personal) and housekeeping included. Emergency call in bedroom and bathroom.

Health care center: Semiprivate rooms in which intermediate care is provided. Respite care offered as available. Registered nurse on duty 24 hours a day. Includes room, meals, housekeeping and nursing care.

Shenandoah Homes Retirement Community: 5300 Hawthorne Road N.W., 362-5412. Sponsored by the First Church of God Virginia General Assembly, but all faiths accepted. Two types of facilities are available:

Adult care: semiprivate rooms (54 beds) are available with care provided by nursing assistants. Meals, housekeeping and linen service are included in the $694 monthly cost. Grants are available. $50 non-refundable entrance fee.

Private apartments: Efficiency (75) and one-bedroom (45) apartments. Meal service is available, but not included. Each apartment is equipped for emergency call service. The monthly fee is on a sliding scale according to the resident's ability to pay. The maximum for an efficiency is $260 per month and the maximum for a one-bedroom unit is $311 per month. There is no entrance fee, but there is a refundable security deposit of $100.

Shenandoah Homes provides van service twice weekly to various shopping areas.

Snyder Nursing Home: 11 N. Broad St., Salem, 389-0160. State-licensed 45-bed facility with licensed nursing staff on duty 24 hours a day. Planned recreation programs. Private pay and Medicaid approved. Semiprivate rooms, $78 per day.

South Roanoke Nursing Home: 3823 Franklin Road S.W., Roanoke, 344-4325. Skilled and intermediate-care nursing home with 104 beds. Private pay or Medicare. Private rooms (14), $84 per day.; semiprivate, $74 per day.

Southern Manor Home for Adults: 3008 Williamson Road N.W., Roanoke, 563-0608. Private, state-licensed adult care facility with 101 beds. Member Virginia Association for Adult Homes. 24-hour supervision. Full-time administrator, L.P.N. and nurses aides. Transportation secured for medical appointments. Meals and housekeeping at a minimum monthly cost of $675. Linen laundry service available at $10 per month.

Valley Care Inc.: 1007 Amherst St. S.W., Roanoke, 342-8861. Residential care for elderly; licensed by social services. 34 private rooms. 24-hour care by nursing assistants. Three meals per day included. Activities, linens and personal laundry, medication management, assistance and supervision provided for daily living. Physicians and podiatrist available. Member Va. Association for Adult Homes. Cost is based on needs of resident.

Valley Home for the Aged: 1418 10th St. N.W., Roanoke, 563-9153. Intermediate-care adult home (no bedridden patients) with 43 beds. Three patients to a room. Nurse's aide on duty 24 hours a day. Transportation to doctor's appointments service secured through MediPort. Kitchen and dining room facilities. Recreation director. Medicaid approved. Licensed by the Virginia Department of Social Services, member of Virginia Adult Home Association. Cost is $675 per month; personal laundry $10 extra per month.

Virginia Veterans Care Center: 4550 Shenandoah Ave. N.W., Roanoke, 982-2860. State-licensed 240-bed long-term care facility. Adult, Alzheimer's, hospice, intermediate and skilled care. Physical, occupational, speech, respiratory and restorative therapies available. Licensed nurses on duty 24 hours a day. Social and recreational activities seven days a week. Van services. Private pay, Medicare, Medicaid and third party payers accepted; VA subsidy available.

Woodhaven Nursing Home: U.S. 460 West, Montvale, 947-2207, (800) 794-2037. State licensed 48-bed nursing home. Alzheimer's care, skilled nursing, I.V. therapy, hospice care, rehabilitation therapy, respite care, as well as long-term care. Licensed nurses on duty 24 hours, 7-day therapeutic, social and recreational activities and van service. Medicare/Medicaid approval pending. Rates from $49 per day.
